Therapeutic Gaslighting Through Assumed Autonomy

Therapeutic gaslighting occurs when a clinician treats structural disempowerment as if it were personal choice. This happens through an assumption of autonomy that only the most socially protected people ever have access to, and then holding everyone else accountable for not exercising it.

In the United States, the group with default access to full relational, social, and economic autonomy is:

  • white
  • male
  • cis
  • straight
  • economically buffered

Everyone else navigates a relational field where autonomy is conditional, punished, or structurally unavailable.

When a therapist says, “You allowed his narcissism,” they are importing a fantasy of autonomy that was never structurally available to the client, and then blaming them for not exercising it. This is a misallocation of responsibility.

People in coercive, religious, or patriarchal relational structures are taught to:

  • submit
  • forgive
  • endure
  • sacrifice
  • stay
  • absorb
  • adapt
  • carry the emotional load

These are the exact traits narcissistic systems exploit. The culture prescribes selflessness and loyalty, while therapy pathologizes those same traits as “codependency.” The contradiction lands on the client, not the therapist.

Therapeutic gaslighting happens when a clinician assumes:

  • choice that wasn’t available
  • safety that wasn’t granted
  • power that wasn’t allowed
  • social backing that didn’t exist
  • exit freedom that wasn’t real

and then interprets survival strategies as pathology.

The therapeutic model used in this situation was built for people who already have the most autonomy in the system. It was never designed for women, queer people, disabled people, people of color, neurodivergent people, trauma survivors, or anyone raised in coercive religious structures.

So when she applied that model to you, she wasn’t treating you. She was disciplining you into a relational ideal that was never designed for you — and blaming you when you couldn’t meet it.
